Daimyo Brazilian Jiu-Jitsu Academy Adult and Juvenile Waiver/Release
NOTICE: THIS IS A LEGALLY BINDING AGREEMENT.

By signing this agreement, I give up my rights for any and ALL causes of action, claims, demands, losses, or costs of any nature whatsoever arising out of or in any way related to me and or my (or children's)) participation or access to any and ALL Daimyo BJJ Academy programs, facilities and related events and activities now or at any time in the future.
I also understand that this agreement is made on behalf of my minor child(ren) and/or legal wards and I represent and warrant to Daimyo BJJ Academies that I have full authority to sign this agreement on behalf of such minor(s).
ASSUMPTION OF RISK AGREEMENT
In consideration of my participation in anyway in Daimyo BJJ Academy activities, I hereby acknowledge, understand and agree that participation in these activities comes with inherent risks that includes but in no way are limited to:
The risk of potential personal injury that could result in permanent paralysis or death, the risk of (a) slips, trips, falls, (b) illness, including exposure to an infection with viruses or bacteria, (c) athletic injuries (d) property loss and/or damage. I further acknowledge that the preceding list is not inclusive of all possible risks associated with my participation in Daimyo BJJ programs and the use of its facilities. I recognize that while particular rules, equipment, and personal discipline may reduce this risk, the risk of serious injury does exist.
I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING FROM THE NEGLIGENCE OF THE RELEASEES or others and I assume full responsibility from my participation and/or my child (ren)'s participation. I understand that I will be solely responsible for all loss or damage. I (my child(ren)) sustain while participating in Daimyo BJJ and its affiliates programs and/or use of its facilities. I further certify that I am (my child(ren) is/are) in good health and that I (she/he) have no conditions or impairments which would preclude my (his/her) safe participation in Daimyo BJJ programs and/or the use of its facilities.
I willingly agree to comply with the stated and customary terms and conditions for participation. If, however, I observe an unusual significant hazard during my presence or participation, I will remove myself (child(ren)) from participation and bring such concerns to the attention of the nearest official immediately.
I, for myself and on behalf of my heirs, assigns, personal representatives and next of kin, HEREBY RELEASE AND HOLD HARMLESS Daimyo BJJ Academy, their officers, officials, agents, volunteers, and/or employees, other participants, sponsoring agencies, sponsors, advertisers, insurers, and if applicable, owners and lessors of premises used to conduct the event ("RELEASEES"), WITH RESPECT TO ANY AND ALL INJURY, DISABILITY, DEATH, or loss of damage to person or property, WHETHER ARISING FROM THE NEGLIGENCE OF THE RELEASEES OR OTHERWISE, to the fullest extent permitted by law.
I understand that as a participant and/or spectator of Daimyo BJJ Academies classes and events, I may be included in videos or photographs taken during the classes/events, and give full release and agree for those videos and photos to be use to promote and/or advertise Daimyo BJJ Academies and its affiliates programs and events, and I also give Daimyo BJJ Academies and its affiliated permission to use/mention my name, if applicable, in any promotion or advertisements of its programs/events.
I HAVE READ THI RELEASE OF LIABILITY AND ASSUMPTION OF RISK AGREEMENT AND I FULL UNDERSTAND AND AGREE TO THE TERMS
